Mise au point clinique et bibliographie
Ce dernier chapitre traduit les six chapitres précédents en conduite pratique. Le principe directeur est simple : l'état de l'épithélium est le seul facteur de risque véritablement discriminant, et la douleur n'est pas un signal d'alerte fiable puisque le produit en cause est lui-même analgésique.
Avant de prescrire
- Examiner l'épithélium à la lampe à fente, avec fluorescéine, avant l'initiation. C'est le seul facteur de risque véritablement discriminant.
- Rechercher activement : syndrome sec et Sjögren (y compris non diagnostiqué — un test de Schirmer et un BUT coûtent peu), polyarthrite rhumatoïde et connectivite, diabète et son équilibre, antécédent d'herpès ou de zona, kératopathie neurotrophique, GVH, kératocône et pachymétrie, rosacée, port de lentilles, chirurgies oculaires répétées ou rapprochées.
- Recenser les co-prescriptions : corticoïde topique, aminoside, antiseptique, anesthésique, collyres conservés au BAK, lentille pansement.
C'est la seule situation où la littérature soutient une proscription et non une mise en garde : 23,6 % d'ulcération ou de perforation dans une cohorte de 140 patients, avec surreprésentation explicite des suites d'AINS, pour un bénéfice jamais démontré[Bourdin 2025]. Le risque persiste tardivement — délai médian de 39 mois après la greffe.
Éviter — kératopathie neurotrophique, melt constitué, sécheresse sévère (contre-indication relative voire absolue selon Rigas[Rigas 2020]), ptérygion associé, large défect épithélial, greffon cornéen.
Pendant le traitement
- Ne pas dépasser 4 instillations par jour et la durée strictement nécessaire. La dose cumulée est un facteur de risque documenté.
- Aucune prescription ouverte ni renouvelable sans réévaluation. Deux des cas les plus graves publiés résultent d'un traitement prescrit une semaine et poursuivi six semaines, ou poursuivi sept mois sans indication documentée.
- Après cataracte non compliquée : 4 à 6 semaines. La prolongation jusqu'à 12 semaines chez les patients à haut risque d'OMC doit être assumée comme un arbitrage explicite, puisqu'elle concerne précisément la population à risque de melt et qu'aucun essai de qualité ne la fonde.
- Privilégier les formes sans conservateur, sans que cette précaution dispense de la surveillance : la patiente 14 de Guidera a perforé bilatéralement en 4 jours sous kétorolac sans conservateur.
- Revoir le patient, et lui donner une consigne écrite de consultation en cas de rougeur, de baisse de vision ou de gêne nouvelle. Ne pas se fier à la douleur comme signal d'alerte.
La fonte sous AINS est pauci-symptomatique et pauci-inflammatoire, parce que l'agent causal est analgésique et hypoesthésiant. Asai décrit des fontes de 60 à 80 % de profondeur avec hyperhémie et infiltration « discrètes ». Le délai diagnostique moyen dans la littérature (≈ 44 jours, extrêmes 3 à 450 jours) reflète cette latence. L'examen à la lampe à fente est le seul dépistage.
Devant une kératolyse suspectée
- Arrêt immédiat de l'AINS — et de tous les topiques non indispensables, conservateurs compris.
- Éliminer une infection : grattages, cultures, colorations. Toutes les séries publiées ont des cultures négatives, mais c'est justement pourquoi le geste est obligatoire.
- Bilan de maladie de système : FR, anticorps antinucléaires, anti-Ro/anti-La, ANCA — le diagnostic de Sjögren ou de polyarthrite a été porté après la perforation dans plusieurs observations, et il change la prise en charge ultérieure.
- Freiner la protéolyse : tétracyclines par voie générale (inhibition non spécifique des MMP), lubrification sans conservateur, sérum autologue, lentille pansement ou recouvrement conjonctival selon le contexte.
- Anticiper le geste tectonique — colle cyanoacrylate, greffe de membrane amniotique, kératoplastie lamellaire ou transfixiante. Un des cas publiés est passé de l'ulcère à la perforation complète en douze heures.
Aucune société savante, aucun RCP n'érige un terrain oculaire en contre-indication absolue. Le dispositif réglementaire repose entièrement sur des mises en garde et sur la surveillance — c'est-à-dire que la responsabilité repose sur le prescripteur. Les seules contre-indications formelles du RCP français du diclofénac collyre sont l'hypersensibilité et l'antécédent d'allergie, d'urticaire, de rhinite aiguë ou d'asthme déclenchés par le diclofénac.
